Abstract
【目的】乳房温存術後に放射線治療を受ける乳がん患者が、放射線治療終了前に体験する複数の症状や症状の背景、複数の症状を体験することによっておこる結果を明らかにする。【方法】5名を対象にインタビューガイドを用いて半構造化面接を行った。複数の症状体験を分析する為にLenzの不快症状の中範囲理論を活用し、まとめた。【結果・考察】患者は、放射線治療終了前に、倦怠感、照射野の皮膚の紅斑、照射野の疼痛、照射野の皮膚の色素沈着、照射野の皮膚の乾燥、照射野の皮膚の掻痒感、照射野の皮膚のヒリヒリ感、照射側の乳房の萎縮、食道周辺の不快感を体験していた。これらの複数の症状は、生じた背景や結果に関連しながら一方向に生じるだけでなく、双方向に影響し合っていた。さらに、患者は複数の症状を体験しながらピアサポートの関わりを持ち、家族と状況を分かち合いながら治療を完遂しようとする等、様々な取り組みを自らが率先して行っていた。この取り組みを支援するためにも、看護師は、症状に早期に介入して、他の症状の出現を予測し、症状に合わせた行動や役割がとれるよう働きかけていくことが必要であることが示唆された。
Aim: The aim of our study was to elucidate the performance of patients with breast cancer who experienced symptom clusters following breast-conserving surgery and prior to the completion of radiotherapy. Methods: Data were collected using a semi-structured interview. We analyzed the records of five patients who experienced symptom clusters using Lenz’s middle-range theory of unpleasant symptoms. Results: Prior to the completion of radiotherapy, some patients experienced fatigue, pain, erythema, pigmentation, dryness, itching, tingling on the irradiated skin, breast atrophy at the irradiated side, or discomfort around the esophagus. Patient prognosis was both positively and negatively influenced, based on these symptom clusters. Conclusion: Therefore, to support the performance in these patients and achieve favorable prognosis, it is recommended that nurses intervene early when patients report these symptoms, foresee related symptoms, and mentor patients in various situations.
