Abstract
目的：化学療法を受ける生殖年齢にあるがん患者の妊孕性ケアに対して、看護師が抱く困難の実態を明らかにする。方法：化学療法看護の経験が5年以上で、妊孕性ケア経験のある看護師6名を対象に、各1回の半構造化面接を行った。分析は、記述データをもとに質的内容分析の手法を用いた。結果：がん患者の妊孕性ケアに対して看護師が抱く困難として、【妊孕性の話題を取り上げる難しさ】【患者と対話を深めて妊孕性のニーズを引き出す難しさ】【がん治療と妊孕性の問題を考慮した判断から、妊孕性ケアにつなげる難しさ】【妊孕性ケアにおける医師・看護師間の協働の難しさ】【施設内・施設間のシステムの問題による妊孕性ケアを行う難しさ】が明らかになった。結論：がん患者に対して、看護師が良質な妊孕性ケアを提供するために、困難の段階に沿って、妊孕性ケアの知識・経験に関する教育的な支援や、専門家につなぐ連携システムの構築、施設内外の妊孕性ケアのシステム構築の必要性が示唆された。
Purpose: The purpose of this study was to clarify the difficulties experienced by nurses providing fertility care to cancer patients of reproductive age undergoing chemotherapy. Methods: The study included six nurses with ≥ 5 years of experience in chemotherapy nursing and experience in fertility care. All nurses completed a semi-structured interview, and interview results were analyzed according to descriptive data using the qualitative content analysis method. Results: The analysis revealed that the nurses experienced the following difficulties in providing fertility care to cancer patients: [difficulty in addressing the topic of fertility], [difficulty in deepening dialog with patients and specifying fertility-related needs], [difficulty in providing fertility care while addressing both cancer treatment- and fertility-related issues], [difficulty in collaborating with physicians in fertility care], and [difficulty in practice of fertility care due to intra- and inter-institutional system issues]. Conclusion: These findings suggest that “educational support regarding fertility care knowledge and experience,” “establishment of a system to facilitate collaboration between nurses and specialists,” and “development of intra- and inter-institutional fertility care systems” in accordance with difficulty levels are required for nurses to provide high-quality fertility care to cancer patients.
