Abstract
本研究は、医療・介護従事者が、がん診断後からがん治療中の患者を支援するための地域包括ケアにおける望ましい医療連携をどのように認識しているかを明らかにすることを目的とした。医療・介護従事者88名に半構成的面接調査を行い、がん治療中の望ましい医療連携に関して語りが抽出された52名を分析対象とし、質的帰納的に分析した。その結果、5大カテゴリ【】に集約された。対象者は、地域包括ケアにおける【副作用マネジメントのためのがん治療医と地域医療従事者の連携】を望み、そのために【拠点病院と地域の医療・介護機関間の診療情報の共有】【拠点病院と一般病院が連携したがん診療提供体制の構築】を必要としていた。また、地域包括ケアの特性を踏まえた【地域で行う心理的サポートの場づくり】【地域で活用できる社会資源の確保】を望ましいと捉えていた。がん治療中の患者を支援するための地域包括ケアによる、副作用マネジメントに係る地域主体の医療連携を築くと共に、心理・社会的サポートを担う地域連携の必要性が示唆された。
Objectives: This study aimed to describe the optimal medical coordination required to support patients undergoing cancer treatment in community-based integrated care systems, from the perspective of health and nursing care providers. Methods: Semi-structured interviews regarding coordination among community-based integrated care systems were conducted among 88 health and nursing care providers. Using qualitative analysis, we analyzed the interview data of 52 participants describing the optimal medical coordination required for patients undergoing cancer treatment. Results: Participants comprised 15 doctors (28.9%), 13 nurses (25.0%), 2 care workers (3.8%), and others. Optimal medical coordination was classified into five main categories. Participants desired "Collaboration between oncologists and community-based health and nursing care providers for sideeffect management" in community-based integrated care systems. They indicated needs for the "Sharing of medical information between designated cancer care hospitals and community healthcare institutions" and "Implementing a system for providing collaborative cancer treatment between designated cancer care hospitals and general hospitals". They further expressed requirements for "Creating a psychological support place in the community" and "Securing social resources for communities" based on the characteristics of community-based integrated care. Conclusion: To optimally support patients undergoing cancer treatment, community-driven medical coordination efforts are essential for the management of side-effects through community-based integrated care, and community health and nursing care providers should collaborate to provide optimal psychological and social support.
